You can request a PLI Certificate in the form below.
All forms will be forwarded to the relevant State Councillors for approval before PLI cover can be provided.
You will be notified when your program/event has been added to our PLI Register and is covered by our Insurance Policy. You should not commence a program/event until you have been notified.
